I acknowledge that there are potential risks, hazards and/or dangers inherent in my participation in the conference (the “Conference”) offered by California Pediatric Dental Research Conference (“CPRC”), including, without limitation, the risk of bodily injury, exposure to or infection with viruses and other microorganisms (including but not limited to COVID19), loss of life and/or loss of or damage to property. I HEREBY VOLUNTARILY ACCEPT AND ASSUME ANY AND ALL RISKS ASSOCIATED WITH MY PARTICIPATION IN THE CONFERENCE, INCLUDING, WITHOUT LIMITATION, THE RISK OF PERSONAL INJURY, EXPOSURE TO OR INFECTION WITH VIRUSES OR OTHER MICROORGANISMS (INCLUDING BUT NOT LIMITED TO COVID19), INCLUDING DEATH, AND LOSS OF OR DAMAGE TO PROPERTY, whether caused by any act or omission of CPRC as the organizer of the Conference. 2. In consideration of my participation in the above described Conference, I HEREBY, ON BEHALF OF MYSELF AND MY HEIRS, EXECUTORS, PERSONAL REPRESENTATIVES AND ASSIGNS IF DECEASED, RELEASE, WAIVE, COVENANT NOT TO SUE AND HOLD HARMLESS CPRC FROM AND AGAINST ANY AND ALL CLAIMS, DEMANDS, CAUSES OF ACTION, DAMAGES, LIABILITIES, COSTS OR EXPENSES THAT I MAY SUFFER OR INCUR AS A RESULT OF MY PARTICIPATION IN THE CONFERENCE OR MY USE OF ANY FACILITIES OR EQUIPMENT PROVIDED BY CPRC, whether or not caused by the negligence (active or passive) of CPRC or others. This waiver and release shall be binding on my heirs, executors, personal representatives and assigns. 3. I further understand and agree that no representations or warranties are or have been made by CPRC, its directors, officers, employees or agents, with respect to the quality or safety of any transportation to be utilized, with regards to any of the accommodations involved, with regards to the supervision provided, or the activities offered during the above described Conference. 4. I understand that this Assumption of Risk and Waiver and Release is required to be signed in order for me to participate in the Conference and is done by me freely and voluntarily with the full knowledge of the risks, hazards, and dangers incident to my participation in the Conference. I agree that my right to participate in the Conference is adequate consideration for the assumption of risk, waiver and release of liability, and hold harmless provided for herein. 5. I further agree that the above Assumption of Risk and Waiver and Release of Liability is intended to be as broad and inclusive as permitted by applicable law. In the event any provision hereof is held to be unenforceable by any court of competent jurisdiction for any reason, the remaining provisions shall not be affected or impaired thereby.
